You must have been abused and have a necessary relationship with the adverse party. 

Examples of abuse include the adverse party: (1) hurting or attempting to physically hurt you, (2) threatening you, or (3) actually or attempting sexual contact with you, without your consent or when you are incapable of consent. 

The necessary relationships include: (1) you are currently dating, (2) you used to date, (3) you currently live together, (4) you used to live together, or (5) you have a child together.
